SMS Appointment Reminders: A Guide for Australian Businesses

Most missed appointments are not deliberate, people forget. If they booked six weeks ago the date likely slipped from their mind, and then the appointment slot sits empty while your staff sit waiting.

An SMS appointment reminder is the cheapest, most reliable way to fix that. What is an SMS appointment reminder?

An SMS appointment reminder is a short, automated text message sent to a customer or patient before a booked appointment. It confirms who the appointment is with, when it is, where it is, and usually gives the person a simple way to confirm, cancel, or reschedule their appointment.

The messages are personalised using merge fields. So your template will pull each person’s name, appointment time, and who they’re seeing from your booking data. You only need to prepare the template once, then the platform can send hundreds of individual, personalised, automated appointment reminders via text.

Do appointment reminder texts reduce no-shows?

Yes, and the evidence is actually stronger than most marketing claims about SMS.

A Cochrane systematic review of mobile phone messaging reminders for healthcare appointments found attendance rates of 67.8% where no reminder was sent, compared with 78.6% where a text message reminder was sent. That is roughly an eleven percentage point improvement from a single text.

The same review found text reminders performed about as well as phone call reminders, with not much difference in attendance between the two. Two of the included studies found the cost per attendance was 55% and 65% lower for text messages over phone calls.

The takeaway for Australian businesses is simple. A staff member going through tomorrow’s bookings and making phone call reminders gets you roughly the same result as an automated text, but at several times the cost.

Which businesses benefit most from appointment reminder texts?

Every business that books time loses something when a customer does not turn up, but it’s not the same thing for every business, that difference should shape how you use reminders.

For a salon or clinic running back-to-back bookings, a no-show is an unrecoverable hour that can’t be refilled. So, the reminder’s job is simply to get the person through the door.

For a healthcare practice with a waiting list, the slot is fillable, but only if you hear early enough to offer it to someone else. In that case two-way replies are a better choice rather than a one-way notification.

For professional services and trades, the loss often runs deeper than the appointment itself. A missed consultation is billable time gone, or a missed site visit loses the travel costs, not just the job.

What to include in an appointment reminder text

A good reminder text answers four questions in as few words as possible: who is it from, when is the appointment, where is the appointment, and what should they do now?

  • Your business name. So, the recipient knows who is contacting them.
  • The date and time, written clearly.
  • The location or who the appointment is with. Often, you’ll want to include both.
  • The next step, such as replying to confirm or calling to reschedule.
  • A contact number for things that a simple reply can’t handle.

Keep it under 160 characters where you can. A standard SMS is 160 characters, and longer messages are split into multiple parts.

An example text that fits comfortably within the 160 character limit could be:

Hi (First Name), your check-up at Baywater Dental is Thu 6 Aug at 2pm. Please arrive 5 minutes early. Reply C to cancel or call 00 0000 0000

When should you send an appointment reminder?

Between 24 and 48 hours before the appointment is the standard. It is close enough that the person remembers receiving it, and far enough out that you might be able to refill the slot if they cancel.

A few situations can call for something different. Bookings made weeks in advance are worth sending two reminders for, one about a week before and another the day before, the first message gives them time to change their schedule if they forgot about the appointment. High-value appointments, or any appointments when you already know the no-show rate is high, can include a short message the morning of the appointment too. On-site work is also often different, the customer will mostly care about the arrival window, and that’s normally sent the morning of the appointment.

You should send the messages during business hours. While the Spam Act does not set restricted hours for SMS reminders the way the telemarketing rules do for phone calls, a reminder that arrives at 9pm can feel like an intrusion and lead to opt-outs.

Two-way SMS: letting people confirm, cancel, or reschedule

A one-way reminder tells someone about their appointment, a two-way reminder tells you whether they are coming. With two-way SMS, replies come back to your platform linked with the original message. A patient who replies to cancel frees that slot early enough for you to try to fill it.

You want to keep the reply instructions as simple as possible. A single character is easier to action than a sentence, and it provides cleaner data on your end.

Are appointment reminders covered by the Spam Act?

This is where a lot of Australian businesses get caught out.

The Spam Act 2003, enforced by the Australian Communications and Media Authority (ACMA), regulates commercial electronic messages. A genuine appointment reminder that contains nothing but factual information is generally not considered a commercial electronic message, which means the consent and unsubscribe requirements that apply to marketing SMS do not apply in the same way.

The exemption is narrower than it may seem at first. If a message has a commercial purpose at all, including through content it links to, ACMA can treat it as commercial no matter how you’ve labelled it internally.

In October 2023 ACMA issued Ticketek an infringement notice of more than $500,000. Part of that finding involved messages Ticketek characterised as factual, but the ACMA determined were commercial because they pointed recipients to content promoting the business.

The practical rule is straightforward, keep your reminders clean and simple. The moment you add a promotional line to an appointment reminder, you have changed your factual message into a commercial one and the full Spam Act regulations apply to it.

Sender ID registration

As of the 1st of July 2026, Australian businesses sending SMS under an alphanumeric Sender ID, a branded name in place of a phone number, must have that Sender ID registered under ACMA’s SMS Sender ID Register. This is just as important for reminders as it is for marketing. Unregistered Sender IDs become labelled as “Unverified” by carriers, which erodes trust in the messages and notifications.
